When you submit the Request a Seat form, we collect the information you choose to provide:
- Your full name
- Your email address
- Your phone number, if you provide it
- Details about your center, including its name, city, number of children enrolled, and years in business
- The message you write about the biggest thing on your plate
Like most websites, our hosting provider may also record basic technical information such as your IP address, browser type, and the pages you visit, for security and performance.
How form submissions are delivered
Form submissions are delivered to us by email through FormSubmit (formsubmit.co), a third-party form service. When you submit the form, your information is sent to FormSubmit, which forwards it to mia@drmiayoung.com. FormSubmit handles your information under its own privacy policy.
How we use your information

- To contact you by email or phone about your request and next steps
- To respond to questions you send us
We do not sell your personal information.
Sharing
We share your information only with service providers that help us operate this site and deliver form submissions, such as FormSubmit and our hosting provider, or when required by law.

We keep form submissions only as long as needed to respond to your request and manage your participation, or as required by law.

This site is intended for adult business owners. We do not knowingly collect personal information from children under 13.
